Patient Timeline and PRISM win Hippocrate awards

Oct 17, 2025

Two innovative projects pioneered by CIUSSS West-Central Montreal have won prestigious Hippocrate awards.

The PRISM program and the Connected Health Record’s (CHR) Patient Timeline took home prizes in the Projet transformateur category at a gala at the Théatre St-James on October 16.

The awards recognize two ground-breaking projects by our CIUSSS. The Patient Timeline revolutionizes access to clinical information by enabling medical staff across our network to obtain up-to-date health information about each patient through a single, easy-to-use digital platform. As a result, staff no longer need to take the time and trouble to consult multiple programs to get a comprehensive view of a patient’s health-related history.

Launched in November, 2023, the Patient Timeline is the first module of the Connected Health Record (CHR), a platform custom-designed for our CIUSSS that provides access to a unified digital patient record of all our 34 facilities.

PRISM (Projet réaffiliation itinérance santé mentale) is a unique program within the Mental Health and Addiction Directorate for adults experiencing homelessness and mental health disorders. PRISM’s interdisciplinary team works in partnership with the Welcome Hall Mission in Montreal to help people at shelters get off the streets, connect with healthcare and community services, and move into their own housing.

“This award is wonderful recognition for our teams, who go to work every morning with the goal of providing optimal care to people experiencing homelessness who require mental health support,” says Laurence Benoit-Dubé, Director of Mental Health and Addiction.

“Along with our partners from different sectors, CIUSSS West-Central Montreal is constantly seeking creative and innovative strategies to let us respond to people experiencing homelessness,” she adds. “This award encourages us to continue thinking outside the box, dreaming big, and pushing ourselves to excel.”

Other projects from our CIUSSS were shortlisted for Hippocrate awards. A software tool known as GrayOS, which optimizes appointment scheduling in Oncology, was a prize finalist. The platform automates the management of Oncology appointments and allows nurses to maximize the use of their time, while reducing down-time for patients.

The Hippocrate awards recognize innovative projects that create value and improve the quality and accessibility of healthcare.
