A sharp rise in Emergency Department visits by people experiencing homelessness has prompted our CIUSSS to launch an innovative pilot project, aimed at fundamentally changing what happens when these patients are discharged.
The Pathway to Recovery initiative transfers eligible patients experiencing homelessness directly to dedicated transitional rooms at the YMCA Tupper, where CIUSSS teams can provide clinical support and connect residents with housing. The teams include a social worker, nurse clinicians, homelessness and addiction liaison workers, and other psychosocial professionals.
